2007 Northwest Urological Society Registration Process

Please make sure to register before November 3rd!
Register before August 3rd and the registration fees are the same as in the past.

CME credits are available for an additional $35.

Registrations received after August 3rd will have $25 added to there registration fees. Members and Senior Members will be charged the guest price for each social event.

1) Print and complete the REGISTRATION FORM or click here to register online.
2) Make your checks payable to the NWUS
3) Mail your completed registration form and payment to:

4) If you are a dues-paid member and do not plan to attend the social events, you are welcome to fax your registrations to the NWUS office at 360-668-4053
